On a separate piece of paper, tell us why the person you nominated is hero.
Include any news clips, police reports, or names of witnesses who can verify
the heroic act and/or depending on the category who can share or collaborate
the impact the individual has had on the life/lives of another Siouxlander.
Award decisions will largely be based on the information you provide.
Please be as detailed as possible.

Hero Categories:

Fire-Rescue
This category honors a firefighter or emergency medical technician has gone above and beyond to save or protect human life in our community.

Law Enforcement
This category honors a local, state or federal law enforcement officer who has gone above and beyond to save or protect human life in our community.

Military
This category honors a uniformed member of the armed services for an act of heroism undertaken in the line of duty, or an extended period of extraordinary service, anywhere in the world.

Good Samaritan (18 and over)
This category honors a person 18 years of age and over for an act of heroism involving an unusual, significant, or unexpected incident requiring a “heroic” reaction in a crisis, or a lifetime commitment exemplifying the values of true heroism.

Good Samaritan (under 18)
This category honors a person under 18 years of age for an act of heroism involving an unusual, significant or unexpected incident requiring a “heroic” reaction in a crisis, or an unusual extended commitment exemplifying the values of true heroism.

Medical
This category honors a member of the medical profession who has displayed an extraordinary commitment to saving life, treating illness or improving the condition of human life in the Siouxland area.

Education
This category honors a Siouxland educator whose commitment to teaching others resulted in a significant impact on the life of a Siouxland individual, educational facility or system.
